Please do not mail your material to the ASR Headquarters. We have had scenarios where you can not upload from work computers so you might have to use a personal computer.

There is a $200 application fee due at the time of submission. You can either use the credit card information field on the application or mail a copy of just your application (so we know who to apply the money to) and your check to: ASR, 7500 Flying Cloud Drive #900, Eden Prairie, MN 55344.
